A Case of Chylopericardium Associated with Tuberculous Mediatinal Lymphadenopathy

Abstract
hylopericardium is a status of chylous accumulation in pericardial sac and is usually associated with mechanical obstruction of the thoracic duct or its drainage into the left subclavian vein.
In chylopericardium, the pericardial fluid is usually milky white with a high choestrol and
triglyceride content, protein content greater than 3.5g/dl, and microscopic fat droplets
demonstrated with a Sudan ¥² stain.
The management of symptomatic chylopericardium consist of ingestion of a diet rich in
medium-chain triglycerides, and ligation of the thoracic duct and parietal pericardiectomy to
evacuate chylous fluid and prevent reaccumulation.
Until recently, only a few cases of chylopericardium have been reported. We experienced
a case of chylopericardium associated with tuberculous mediastinal lymphadenopathy in 17
year old female. And improvement of pericardial chylous effusion was noted by low fat diet
and antibuterculous chemotherapy without surgical intervention such as pericardiectomy or
thotacic duct ligation.
